PHECC Affiliated Trainers

Full procedures are provided in the Trainer Policy & Procedures Folder.

Affiliated Trainers must complete this PHECC Faculty Form and submit it be email along with copies of relevant certificates and evidence of insurance.

Suitable venues must be selected in accordance with the Premises Selection Procedure and confirmed on the Course Notification & Pre Check Form which must be completed and sent on at least a week before course commencement.

The Venue Checksheet part of the Trainer Report must be completed on the day of the course.

Course results must be submitted by email within a week of the course on the Trainer Course Report along with the signed Learning Agreement, evaluation forms, attendance Sheet, room set up pictures, Skills Sheets and MCQs.
