Affiliated Trainers must complete this PHECC Faculty Form and submit it be email along with copies of relevant certificates and evidence of insurance.
At least a week before the course the Course Notification and Pre Course Checksheet must be completed and sent on.
Prior to the commencement of the course the Venue Checksheet must be completed.
Course results must be submitted by email within a week of the course on the Trainer Course Report contained in this folder along with the Learning Agreement, evaluation forms, attendance Sheet, Skills Sheets and MCQs.
